This website (www.marionpolkdental.org) is provided by the Marion Polk Dental Society (“the Society”), located at 4676 Commercial St SE, PMB 502, Salem, Oregon 97306.  Please read the Terms of Use carefully. Each time you access, use, or download materials from this website, you agree to be bound by these Terms of Use and conditions and any updates hereto.  If you do not agree to be bound by these Terms of Use do not use this website.

7. If the Society engages a third party to perform services on behalf of the Society, your personal information (for example, e-mail, name, address, telephone number) may be provided to the third party to conduct the service, but it is not transferred to the third party for any other use.

8. Do not communicate private, confidential or privileged information using this website. Please be aware that e-mail communication can be intercepted in transmission or misdirected. Email and contact forms are not considered to be completely secure. Please do not include personal identifying information such as your birth date, or personal medical information in any emails you send to us.
