This website (www.marionpolkdental.org) is provided by the Marion Polk Dental Society (“the Society”), located at 4676 Commercial St SE, PMB 502, Salem, Oregon 97306.  Please read the Terms of Use carefully. Each time you access, use, or download materials from this website, you agree to be bound by these Terms of Use and conditions and any updates hereto.  If you do not agree to be bound by these Terms of Use do not use this website.

 

Your use of this website and access to the information is expressly conditioned upon your agreement that all such access and use shall be governed by these Terms of Use.

Links may be made to this website provided the links do not use the trademarks or service marks of the Society or any affiliated entities.  Links to this website may not suggest or imply that the Society or its affiliates support the linking website, its sponsors, affiliates or advertisers, unless prior written permission to indicate such endorsement or support has been granted. 

  2. This website is a dynamic website and changes are made regularly.  The information provided is provided without warranty of any kind.   6. The society strives to protect your privacy. The Society collects and uses your personal information to operate this website and to deliver the services you and/or its members have requested. The Society does not sell, rent or lease its customer lists to third parties, but the Society does share its membership information with the Oregon Dental Association and the American Dental Association. The American Dental Association maintains a membership database with member information.   7. If the Society engages a third party to perform services on behalf of the Society, your personal information (for example, e-mail, name, address, telephone number) may be provided to the third party to conduct the service, but it is not transferred to the third party for any other use.

  8. Do not communicate private, confidential or privileged information using this website. Please be aware that e-mail communication can be intercepted in transmission or misdirected. Email and contact forms are not considered to be completely secure. Please do not include personal identifying information such as your birth date, or personal medical information in any emails you send to us.

  9. The Society may disclose any information held by the Society about you, without notice, if required to do so by law or in the good faith belief that such action is necessary to: 

    1. comply with applicable law;

    2. enforce the Society’s rights; or

    3. act under exigent circumstances to protect the personal safety of others.

  10. The Society grants you a non-exclusive and non-transferable license to use this website for your personal, non-commercial purposes only. You may download, view, copy, and print the information incorporated into this website solely for your non-commercial use. The information may not be transferred, shared with or disseminated with anyone for any purpose which is inconsistent with the purpose of the website, to facilitate unfair competition with the Society or this website, or for any purpose which is unlawful.

During monitoring, material may be examined, recorded, copied, and used for the Society’s purposes. Use of the website, authorized or unauthorized, constitutes consent to such monitoring. Cookies are deployed on this website thereby improving the quality of service. No guarantee can be made by the Society as to the correct functioning of this website should cookies be disabled in the browser being used. Cookies enable the Society to identify your device, or you when you have logged in. We use cookies that enable you to move around the site or to provide certain basic features and to enhance the functionality of the website (by storing your preferences, for example). We do not sell the information collected by cookies, nor do we disclose the information to third parties, except where required by law (for example to government bodies and law enforcement agencies).

If the Society takes action to enforce any of the provisions of these Terms and Conditions, the Society shall be entitled to recover from you (and you agree to pay), in addition to all sums to which it is entitled or any other relief, at law or in equity, reasonable and necessary attorney’s fees and any costs of any litigation.

  15. These terms shall be governed by the laws of the state of Oregon, excluding any rule or principle that would refer to and apply the substantive law of another state or jurisdiction.
